TRIPIL CIL'S Five Core Services
Nursing Home Transition (NHT)

Tri-County Patriots for Independent Living (TRIPIL) works with people residing in nursing facilities who wish to move back to the community. TRIPIL along with local partners identify and assist people under the age of 60 to make an informed decision to move out of nursing facilities into the community of their choice.

NHT workers identify the person's fears about moving back to the community and work with them to remove or overcome them if possible. Most often these fears include accessible affordable housing or home modifications, transportation, personal attendant services in their home, equipment or assistive technology, and the availability of funding outside of a nursing facility. Overcoming these fears involves honoring the wishes and direction of the person trying to relocate in order to find and coordinate the services specific to their needs.

For the person to make the move out of a facility into their own home, apartment or family/friend/foster care arrangement, the

provides for one time expenses and in-home care based upon that person's income and disability.
