Nursing Home Transition Services
"Our Homes; Not nursing homes" has been our campaign for Advocacy as well as services. Trained Volunteers and Staff frequently visit nursing homes seeking people who want to rejoin the community. Under consumer direction, we find the right combinations of assistive technology and services to realize freedom and independent living.

Assisting individuals to find affordable, accessible housing options & home modifications.

Training to improve functional living skills including Home & Financial Management, Social & Communication Skills, Managing Your Health, Managing a Personal Attendant, Pre-Vocational Skills, Community Rights & Responsibilities, and many others.

Peer Support
Persons with disabilities aid one another through self-help and problem solving together.
